1. Applicant’s NRIC (Front and Back)
2. Applicant’s Birth Certificate
3. 1 recent coloured passport size photo (400 x 514 pixels in size taken within the last 3 months, with white background in jpg, jpeg, png or gif)
4. Latest education result transcripts
5. Eyesight prescription if you wear spectacles/corrective lenses
6. Deed poll for name (if applicable)
7. New Citizenship Certificate (if applicable)

Step 2: When you are ready to attempt the online application form, please get one of your parents/legal guardian to sit through with you as we need their signature at the end of the form.

Step 3: Click “here” to apply. Please note that the online form will take about 20 minutes to complete. The form should be completed in one sitting as there is no option of saving the form.

• Freshmen preferred
• Healthy and no major illnesses
• Height – 1.62m to 1.90m and healthy BMI
• Vision:
  • Myopia – below 800 deg each eye
  • Astigmatism – below 300 deg each eye
  • Normal colour vision
  • No other eye deficiencies
